Before joining the practice all new patients will be asked to fill in a medical questionnaire and will be expected to attend a compulsory new patient appointment. Having this information will make medical staff better aware of your needs.
If you wish to register at the practice, please pick up a registration and patient health form at the reception desk, or download and print them at home.
You will be allocated your doctor. This named doctor is for legal purposes only, and you can choose to see any doctor (subject to availability) of your choice by pre-booked appointment.
