Sick/Fit Notes

New Fit Note requests are available until 09.30am daily only.

New Fit Notes can only be requested after 7 days off work.  Before that please use Self Certification process as outlined by your employer or use this link here. Employee’s statement of sickness to claim Statutory Sick Pay - GOV.UK (www.gov.uk)

Fit Note extensions are available to submit at any time.

A GP may call you at any time during the day if they need to discuss request with you.

In both instances you will be contacted as soon as the Fit Note is ready for collection.
